I understand that the Brunswick High School Athletic Rules require that students ride the bus to and from all athletic events and a departure from this requirement will release the Brunswick School Department from all liability with reference to the above stated transportation.

This form must be signed by the Athletic Director prior to the dismissal of school or early dismissal of teams on the day of the contest. The form must be given to the coach before the student is allowed to leave the contest.
